In some cases the patient and family feel adrift and at discharge leave the hospital or rehab facility with many unanswered questions at a time when they feel most vulnerable or fragile. This should not occur. A physician-advocate can help ensure that such transitions are smooth, that care has been appropriate and coordinated, and that you or your loved one are confident and clear about the days ahead. A physician-advocate can also help clarify treatment options in complex cases during in-hospital care if you feel your team of physicians may not be communicating in an ideal fashion.
